you would like to save your details and make amendments. Click SUBMIT after you are
satisfied with the information given. Any amendments after clicking SUBMIT will
not be allowed. |

Your
application is recorded in the system once the message " Thank
you. Your application has been recorded" appears on the
screen. Please click on the "PRINT" BUTTON to download a copy
of your application. |
6. |
Please send the printed application form and the relevant certified
documents in duplicate which should include: |

- Two (2) copies of application
forms completed for all sections, signed and attached with
recent passport-sized photographs on each form;
- Two (2) certified true
copies of your degree/diploma;
- Two (2) certified true
copies of official transcripts;
- Two (2) copies of referee
form from two (2) referees;
- Two (2) copies of financial
guarantee letters from sponsor or two (2) financial guarantors;
- Two (2) copies of your
research proposal for programme by thesis only (1,000 -
1,500 words for Masters, 1,500 - 2,000 words for PhD).
- Two (2) photocopies
of your passport (for international applicants only)
- Two (2) postcards with
name and self-addressed with postage 50 cents for local;
- Processing fees of
RM20 (for Malaysian applicants) or bank draft of USD30 (for
international applicants) made payable to Bendahari UKM
or bank-in the payments to BCB UKM No. 1218-0005004-05-8
(Please enclose the copy of the bank slip).
